• Payment: to bill and collect payment for services, including deposits and memberships.

• Appointment reminders and service updates about care you have booked.
• As required by law, for public health and safety, or in response to a valid legal order.

Uses that need your written permission

We will not use or share your health information for marketing, sell it, or share before-and-after photos publicly (for example in a gallery or on social media) without your written authorization. You may withdraw an authorization at any time, except where we have already acted on it.

Your rights

• See and get a copy of your health information. You can download your records at any time from the Privacy & My Records page of your account.
• Ask us to correct information you believe is wrong or incomplete.
• Ask for a list of the times we have shared your information outside of treatment, payment and our operations.
• Ask us to limit what we use or share, and to contact you in a specific way or at a specific address.
• Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
• Choose someone to act for you, such as a legal guardian.
• File a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights were violated. We will not retaliate against you for filing a complaint.

Our responsibilities

We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information. We will not use or share your information other than as described here unless you tell us we can in writing.

Changes to this notice

We may change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available in your account and on request.
